A. An interdisciplinary collaborative approach to rehabilitation and goal setting is recognised as best practice.
B. Good team practice includes:
- Focus on participation-level goals
- Emphasise the involvement of the client in goal-setting .
- Ensure all clinicians have conversations with the client about the client’s goals.
- When necessary have more than one meeting with the client to identify their goals – dependent on client age, nature and severity of injury and adjustment to injury
- Use rehabilitation plans are part of a goal setting and review process
- Use team meetings or case conferences review client goals and assessment results as well as develop action plans Interdisciplinary collaborative approach
